How to use this template

Read the whole thing before you edit. Some of the wording is deliberately careful — for example, "appreciate the opportunity to review" is neutral acknowledgment, not agreement or admission.

Replace every bracketed field with your specifics. If a clause doesn't apply — for instance, if the plan already contains clear measurable criteria — you can remove it. Keep the tone consistent.

Send by email to your manager, cc your HR business partner, and save a copy for yourself outside company systems. A PDF attachment preserves formatting.
