Given my tenure at [Company] and the timing of this transition, I would like to request severance of [X] weeks of pay (approximately $[Amount]). This reflects standard practice for comparable roles and gives me a reasonable runway to secure my next position.

I would also like written confirmation that all accrued and unused PTO will be paid out with my final paycheck.

To help bridge my healthcare during this transition, I would appreciate three months of employer-paid COBRA premiums, or an equivalent lump sum added to the severance.

I would appreciate a neutral written reference letter confirming my dates of employment, title, and scope of work, which I can share with prospective employers.

How to use it
